In Dr. Mario, players assume the role of Mario, depicted as a doctor, tasked with eradicating various red, blue, and yellow viruses proliferating within a medicine bottle.

The core gameplay revolves around dropping two-colored vitamin pills from the top of the screen. Players can move and rotate these pills as they descend. The objective is to align four or more blocks of the same color (comprising both pills and viruses) either horizontally or vertically. Once this condition is met, the matched blocks disappear.

Pills can split into two halves upon impact or when they fall, creating strategic opportunities, especially for filling small gaps or clearing hard-to-reach viruses.

The game features multiple difficulty levels, adjusted by increasing the number of viruses and the speed at which pills fall. As players advance, the challenge intensifies, demanding quick reflexes and strategic thinking.

Beyond the single-player mode, Dr. Mario also includes a competitive two-player mode. In this mode, players race to clear their viruses faster than their opponent. When a player completes a large combo, garbage blocks can be sent to the opponent's screen, hindering their progress and adding a layer of competitive intensity.
