The gameplay of Ultra Toukon Densetsu focuses on a blend of physical strikes and grappling techniques. Players use the joystick to move their wrestler and buttons to perform actions such as punches, kicks, grapples, and holds. When wrestlers get close, they can initiate a grapple, where players must quickly input commands to execute moves like suplexes, slams, or various throws. Timing and precision are key to success in these grappling exchanges.

Each wrestler has a health or stamina bar, which depletes as they take damage. When this bar is low, the wrestler becomes more susceptible to pins or submissions. Additionally, the game features a "momentum" or "spirit" system that allows wrestlers to unleash powerful finishing moves when the bar is full. These finishers often deal significant damage and can lead to the match's conclusion. To perform a pinfall, players must knock down their opponent and then press a button to pin them. The pinned wrestler must attempt to "kick out" by mashing buttons before the referee counts to three. Similarly, during submission holds, the trapped wrestler must find a way to escape by button mashing or crawling to the ropes.

The game features 2D graphics, but the wrestler models are quite detailed, and animations are fluid, faithfully recreating NJPW's characteristic moves. In-game audio includes impact sounds, commentary, and energetic background music, contributing to the intense atmosphere of a professional wrestling match.
