Each stage in TeddyBoy Blues is divided into several smaller areas. Players move TeddyBoy back and forth across platforms, shooting a variety of strange and whimsical monsters that appear from all directions. These enemies range from flying creatures to jumping animals, all aiming to stop TeddyBoy. When shot down, enemies can transform into money (Zeni) or power-up items.
A unique aspect of TeddyBoy Blues is its currency system and shops. Players collect coins not only to increase their score but also to purchase upgrades for TeddyBoy at special shops that appear scattered throughout the levels. At the end of each stage or a series of stages, players will face a boss battle. However, boss battles in TeddyBoy Blues are not always direct confrontations. Instead, players often need to collect a certain amount of coins within a limited time, while dodging the boss's attacks. Completing this objective unlocks the next stage.

The game is named after and features a soundtrack based on the popular pop song "Teddy Boy Blues" by Japanese idol Yoko Ishino, giving the game a very distinct and vibrant style.
